Forms and Registration

 

Registration is easy. Simply download the following form and complete the required fields. Payment can be made by credit card. Once you have completed this form, please send it to us via e-mail or post if you are sending a check (both addresses are available to you on the form). Please note that a $50 fee is assessed for late registration.

Continuing Education Registration Form

Financial Aid, although limited, is available to those in need of tuition support. This form, in conjunction with your tax documentation, is required. Please send all completed materials to us on the address listed on the form.

Transcripts can be requested with the online Continuing Education Transcript Request Form. Each transcript requires a $6.00 fee. Transcripts cannot be issued for students with outstanding fees or fines.

Add/Drops can be completed with the online Continuing Education Add/Drop Form. For course withdrawals, a refund will be given based on the formula in the Continuing Education Catalog. Notifying an instructor of your intent to add or drop is not sufficient and does not constitute enrollment or withdrawal from a course.

Please complete an online Continuing Education Change of Address Form so that we always have the most recent information on record.
