New England Conservatory International Student Forms

Transfer to New England Conservatory
Use this form if you are already in the US on an F-1 visa sponsored by another school. This form should be signed by your current International Student Advisor and then faxed or mailed to the International Student Office at NEC.

Check-in
This form should be completed and returned within the first two weeks of every semester.

Program Extension
If you will not graduate on time you should complete this form and have your advisor sign it.

Curricular Practical Training
This packet contains all of the information you will need to apply for Curricular Practical Training. The Faculty Recommendation Form should be signed by your studio teacher after you have received a written off-campus job offer which details the dates of your employment and the duties which you will perform.

Department of Homeland Security Forms

Change of Address
All student visa holders are required to notify the Department of Homeland Security within ten (10) days of changing address. You may downoad this form and send it to the address provided, or change your address online. Please note, you must also notify the International Students Office of any address changes.

Optional Practical Training
Your OPT application must be authorized by an Internatinal Student Advisor before it can be mailed to the Vermont Service Center for Department of Homeland Security processing. Stop by the office for a complete package of informaiton about OPT.

Lost I-94 Arrival/Departure Card
This is the small white card that was stapled in your passport when you entered the US. You should keep it in your passport at all times, because it verifies your status in the US. If you lose this card, download the replacement form here.

Change of Visa Status
If you decide to change your visa status (i.e., from F-2 to F-1), you should apply directly to the Deparment of Homeland Security using this form.
